Reporting side effects

Every report of a side effect makes medication safer, for you and for other patients. This page explains how to report a suspected side effect and what we do with your report.

In an emergency If you experience severe symptoms, such as difficulty breathing, swelling of the face or throat, or loss of consciousness, call the local emergency number or contact a doctor immediately. Do not wait to submit a report first.

1.What is pharmacovigilance?

Pharmacovigilance is the continuous monitoring of the safety of medicines. As a pharmacy, we record and assess reports of suspected side effects and, where required, share them with the responsible authorities and manufacturers. This is a legal obligation and an essential part of safe pharmaceutical care.

2.How to report a side effect to us

  • Via the contact form on this website, selecting the subject Side effects (Pharmacovigilance).
  • By email to contact@eurocarepharma.com.
  • By phone on +31 (0)85 333 2687, Monday to Friday, 09:00 to 17:00 CET.

3.What to include in your report

  • The name and strength of the medication.
  • The batch or LOT number and expiry date, if available on the packaging.
  • A description of the reaction, when it started and how long it lasted.
  • Other medicines you use, if any.
  • Your contact details, so we can reach you for any follow-up questions.

4.What we do with your report

  • Your report is recorded and assessed by a pharmacist.
  • Where required, the report is forwarded to the marketing authorisation holder and the competent authorities.
  • We may contact you for additional information.
  • Your personal data is handled confidentially, as described in our Privacy Policy.

5.Reporting directly to the national centre

You can also report side effects directly to the national pharmacovigilance centre in your country. Reporting to us and to the national centre are both valuable and do not exclude each other.

6.Product quality concerns

Concerns about the quality of a product, such as damaged packaging, can be reported in the same way. Do not use the medication until you have spoken to us or to your healthcare provider, and keep the product and its packaging.
